Most forms and petitions are in Portable Document Format (PDF) and require Adobe Acrobat Reader.  If you do not have Adobe Acrobat Reader, you may download it free of charge.

With the exception of the Tele-BEARS Schedule Form, you may not submit forms or petitions to the Engineering Student Services office front desk or any other staff member, as they are not able to evaluate nor vouch for its completeness. Tele-BEARS Schedule Forms may be returned to the appropriate box at the Engineering Student Services office in 308 McLaughlin Hall.
